Dr. Salas is a physician trained in Physical Medicine & Rehabilitation with advanced specialization in interventional spine and sports medicine. Surgical residency training at the University of Miami and Yale gives him a strong anatomical foundation for image-guided and minimally invasive procedures.
He practices interventional spine and musculoskeletal medicine with the Miami Institute for Joint Reconstruction, sees functional and longevity patients at Aliestetic, and is co-founder, owner and head programmer of CoreMotif Functional Training Center. A former member of the Venezuelan national rugby team, he knows what it takes to train through, recover from and come back after injury. His approach is outcome-driven: accurate diagnosis, minimally invasive intervention, functional restoration and long-term durability.

Regenerative medicine
Platelet-rich plasma (PRP) and bone marrow aspirate concentrate (BMAC) are prepared from your own blood or bone marrow in the same visit, then placed precisely with ultrasound or fluoroscopy into the tendon, ligament or joint that needs it.
Platelet-rich plasma
Platelets carry growth factors, including PDGF, TGF-β and VEGF, that signal the body’s tissue repair response. PRP concentrates platelets several times above normal blood levels and delivers them directly to injured or degenerating tissue.
A small amount of blood is drawn from your arm, like a routine lab test.
The sample is spun to separate red cells, white cells and platelet-rich plasma.
The PRP is prepared for the target: leukocyte-poor for joints, leukocyte-rich often for chronic tendon problems.
Ultrasound or fluoroscopy places it exactly into the tendon, ligament or joint.
Bone marrow aspirate concentrate
BMAC is made from bone marrow drawn from the back of the pelvis. Once concentrated, it contains growth factors, signaling proteins and a small population of mesenchymal stromal cells, a richer biologic mix than PRP.

Straight talk
PRP has its strongest support in knee osteoarthritis and lateral elbow tendinopathy, where randomized trials show improvements in pain and function. Evidence for other tendons and joints is growing but more mixed.
BMAC research is promising, with smaller studies so far. Neither treatment rebuilds a worn-out joint, and results vary between people.
That is why regenerative treatment here is one part of a plan that includes an accurate diagnosis, image-guided placement, load management and progressive rehabilitation.
What to expect
Pause anti-inflammatory medications as directed. Hydrate and eat normally.
Outpatient procedure. You go home the same day.
Soreness is expected: it is part of the healing response. Relative rest.
Guided rehab progression back toward training.
Improvement typically becomes noticeable. Progress is reassessed.
Full effect. Decide together whether a repeat treatment makes sense.
Candidates
Good candidates often have persistent tendon, ligament or joint pain despite physical therapy, mild to moderate arthritis, or want to delay or avoid surgery.
Not suitable with active infection, active cancer, certain blood disorders or very low platelet counts. Blood thinners and recent steroid injections need planning first. PRP and BMAC are generally not covered by insurance; pricing is discussed at your consultation.
PRP contains no stem cells. BMAC contains a small population of mesenchymal stromal cells alongside growth factors. Be cautious of clinics that promise stem cells will regrow cartilage; the benefit is better understood as a biologic signal that supports healing.
The injection is done with local anesthetic and image guidance. Soreness for a few days afterward is common and expected. BMAC also causes soreness at the harvest site.
Many patients need one to three PRP sessions. BMAC is usually a single procedure. The plan depends on the diagnosis and how you respond.
Yes, with a plan. Training is modified for the first weeks, then progressed. This is where the rehabilitation and training background matters most.

A physician-led sports medicine, functional and performance evaluation at the start and end of every training cycle. Training programs are designed for populations; this consult tests their assumptions for one person and changes the plan where they do not hold.

The consult is an evaluation and planning service. Imaging, lab testing and interventional or regenerative treatments are not part of it; when indicated, they are discussed and scheduled separately. Performance and body composition testing may be performed by separate testing providers.
Co-founder, owner & head programmer
A functional training center in Miami delivering coached group training through a structured, periodized system. Every class belongs to a weekly movement split, every week belongs to an 8-week mesocycle, and every mesocycle ends with testing. Programming is designed and overseen by Dr. Salas.

Program
The core program, Monday through Saturday. A 60-minute coached session with activation, strength, core and accessory, and conditioning. Scaling is written in, so every level shares the same session.
